Assessing patients in the community requires the skills to examine, not just the patient, but the environment in which they live and in which care will be delivered. Delivering compassionate care as outlined in Compassion in Practice (2012) will underpin this module.
Comprehensive holistic assessment is key to the delivery of high quality and cost effective care to patients in the community. Assessing patients in the community requires the skills to examine, not just the patient, but the environment in which they live and in which care will be delivered.
Our module will enhance your practice, providing the knowledge and skills to enable you to research the evidence and deliver the most appropriate care. You will develop patient-centered, system-based assessment skills which will support accurate clinical decision making.
Delivering compassionate care as outlined in Compassion in Practice (2012) will underpin this module. You will enhance your skills when undertaking an assessment of circumstances, in which the patient lives to ensure the appropriate care is delivered by the right service. This module will support entry to the non-medical prescribing module. When combined with the non-medical prescribing module, this module enables future community practitioners to contribute to the government agenda of caring for people closer to home, to reduce unnecessary hospital admissions.
You are required to have a practice placement and be under the supervision of a GP or a Nurse Practitioner with the relevant clinical assessment skills.
Assessment
The assessment task for this module will involve a formative practice assessment, the presentation of a case study and the successful completion of a clinical competency framework.
