This module is the compulsory module for the Diploma of Higher Education in Long Term Conditions as the key concepts discussed within it are fundamental to the care of people living with long-term conditions.

By studying Education for Health’s Holistic Care for People Living with Long Term Conditions module, you will gain the knowledge, skills and attitude to confidently support and manage people living with chronic disease.

This module covers the following areas:

· Recognising the burden of long term conditions

· Recognising the impact of long term conditions on patients

· Supporting self care for patients with long term conditions

· Delivering effective disease management for long tem conditions

· Supporting people with terminal illness

· Professional development

By the end of the module you will be able to:

· Discuss the nature and perceptions of long term conditions management

· Assess and examine the principal issues for health professionals in managing people with long term conditions and apply these to different situations

· Critically examine the principles of patient- centred care in long term conditions management

· Discuss the principal issues for people who are living with and managing long term conditions and those of their carers in order to develop practices which promote choice, well being, reduce the risk of complications and improve quality of life.

· Examine the principles of long term condition management in relation to disease specific issues in the development of their own practice

This module will take 4 months to complete and is delivered through comprehensive study materials supported by 1 study days and submission of a 2,500 word assignment.
