Course programme

Understand the enablers and barriers to the delivery of integrated care and how to navigate this complex model of care delivery. Our module looks at integrated care in its broadest sense from both an international and national perspective, examining lessons learnt from different models of integration in health and care systems.

People are living longer with complex conditions and cross system, complex care models are being developed as part of the Government’s Five Year Forward Plan (2014), impacting on the whole health and social care system and bringing rising costs and heightened patient and carer expectations. We need to change the way health care is delivered, both in the UK and abroad, via integrated services in to deliver quality, person-centred care. Practitioners face many challenges involved with integration, including communication, different organisational and professional cultures and organisational challenges such as different information technology and budgets.

This course will help you to develop the knowledge you need to understand the concept of integrated care, with opportunities to apply your learning in practice. You’ll critically analyse underlying theory and how it contributes to the delivery of patient and client care in the current political climate. You'll also gain an insight into some of the governance and legal and ethical issues involved in delivering integrated care.
