At the end of this course, candidates will have the knowledge to be able to carry out an accident investigation. The primary purpose of any accident investigation should be to gather information in order that the facts can be determined with a view to preventing a recurrence. It should not have as its primary objective, the determination of a scapegoat. Blame may indeed have to be apportioned, but that is a matter for the investigation report and is dependent upon the facts and contributory causes as they are revealed in due course. Course programme
The primary purpose of any accident investigation should be to gather information in order that the facts can be determined with a view to preventing a recurrence. It should not have as its primary objective, the determination of a scapegoat. Blame may indeed have to be apportioned, but that is a matter for the investigation report and is dependent upon the facts and contributory causes as they are revealed in due course.
All accidents, including near-miss and minor first aid events, should be investigated. The fact that no injury or damage, or only minor injury or damage has occurred on this particular occasion, does not mean that a more serious injury/damage potential does not exist for the future. The seriousness of an accident is only a matter of degree; a small cut to a finger on a particular occasion might, if circumstances were changed slightly, have resulted in the loss of a hand
