This course will cover the management of long term conditions and their monitoring in general practice. In particular you will develop your knowledge and skills in a wide range of conditions including: asthma, cardiovascular disease, chronic obstructive airways disease, diabetes, hypertension, dementia and frailty and 'fatty liver disease'. Through a critical understanding of medicine management and working with patients you will develop an understanding of the general practice nurse role in these areas.

You will benefit from a combination of University teaching and learning in practice with Mentor guidance. Through critical learning you will consider issues relating to leadership, team working, quality and governance, and accountability in order to develop you in your role of the general practice nurse.
Covering political issues, NHS strategies and developments in primary care you will gain invaluable knowledge and understanding of the key issues of your role.

Module Content
Within this module you will gain further knowledge on a wide variety of topics, essential to general practice nursing. This includes:
- consultation skills
- health assessments
- diagnostic tests
- health screening
- long term condition monitoring and management
- self care planning
- medicine management
- working with individuals who have a variety of health needs.
The module will also cover:
- patient as a partner
- team working and leadership
- record keeping and IT in relation to clinical systems
- quality monitoring and assurance
- ethical and legal issues, negligence and employment law.
How will I be taught and assessed?
A variety of teaching methods are used within the programme including group work, self directed learning, demonstrations and tutorials. Online learning will be used to complement traditional methods of teaching.
The assessment of the module will be through a summative report based on a patient from general practice where you have been involved in the management of their long term condition.
The credits from this module can be used towards an honours degree qualification in BSc (Hons) Professional Practice (Healthcare).
