Teams introducing care coordination encounter a range of challenges, some of which may deter the team and undermine the success of the programme. This training will enable teams to navigate these and other difficulties and realise the full potential of care coordination for patient care and team motivation. This blended learning programme offers guided learning activities, one to one tutor contact, and assessment of written work, for the preparation and introduction of care coordination systems.
The benefits of running a care coordination programme are becoming increasingly clear in terms of their potential to enhance the patient’s dental experiences and their perception of the quality of their care. To reach its full potential care coordination needs to be embedded into the practice’s operational strategy. This course is for dental teams planning to introduce care coordination services and will help them to plan well- led, effective and caring care coordination services.
