Size, Site, Wound History, Condition of the wound (bed, Edge, surrounding skin) Evidence of Infection, Odour, Pain, Fluid, Exudates / Pus / Serous fluid, Intrinsic & Extrinsic factors affecting healing, Previous related wound management regimes including success/ failure & how long they were used.
