Describe factors that impact on person centred care
Recognise the need to involve the service user in care planning
Produce concise care plans to promote safe practice and compliance
Measure the effectiveness of the care plans in line with CQC outcomes
Demonstrate the benefit of completing effective care plans by the care you deliver

Managers, senior leaders and employees from all types of care establishments who want to develop effective care plans in order to achieve care compliance.

This is an interactive session and delegates will have the opportunity to use and critique various methods of Care planning.

Delegates will have the opportunity to reflect on the key elements that are frequently targeted by the CQC and Local Authority:
Compliance - how do you put safe dignified care into practice and fulfil obligations set out by Local Authority and CQC whilst striking a balance with the care you deliver?
Consent - how do you obtain, record and incorporate into your person centred approaches?
Choice - respecting and measuring the importance of choice, whilst taking into consideration safety and legal influences
Capacity - how does capacity impact on the care we deliver and how should this be recorded?
Consistency – the importance of concise care planning and to explore factors that influence consistency and the impact this has on care delivery
Competency - how competency plays a role in positive and negative outcomes of care.

Care planning is the responsibility of anyone involved in the care delivery process from those devising the care plans to the front line staff responsible in meeting the daily interventions of the service users.
