Failure Modes and Effects Analysis is a step-by-step process for identifying all of the possible failings in a process, product, service or new design. The tool is used to help practitioners predict what might go wrong – and how bad the effect of that failure could be – so that plans can be put in place to prevent them from occurring or to detect them before incident.
Failure modes means all of the ways a process, product, service or new design could fail, while effects analysis considers the scale and consequences of those failures.

Finding faults or failures in a product, service or new design once it is completed and on its way to the customer is expensive, damaging and potentially dangerous. An FMEA helps practitioners to find likely causes of problems before they occur so it never gets to that stage.
