Long Term Conditions Foundation Degree - Awarded by St George's University

Course content:
This course is aimed at aspiring associate practitioners to help extend and develop their role and help contribute to improving and understanding the delivery of patient care.
The Foundation Degree in Long Term Conditions will offer you an opportunity to develop a portfolio of knowledge, skills and a level of professional maturity critically different to that of current health care support workers. It will enhance your existing skills and enable a heightened role within and across health and social care to support the needs and management of people with long term conditions.
This course will enable you to develop core knowledge and skills pertinent to working with people living with long term conditions. It includes specific unties of learning development to enable you to consolidate and increase understanding through application to a specific work place setting.
the course consists of 15 module, examples of which are listed below. Please note that these are indicative and not a definitive list. Those listed here may also be a mixture of core and optional modules.

Modules can be accessed on an individual basis as part of the continuing professional development portfolio for health care support workers and associate practitioners.

Course Structure:
Flexible, blended learning approach which includes face-to-face teaching, online delivery, a portfolio of learning and work based learning to support and complement students' learning in a practice based environment.
The course has been designed by a unique partnership between The Faculty of Health and Social Care Sciences at St. George's University of London and Kingston University, South Thames College and South West London Academic Network and offers a wide variety of progression pathways.

Course resources:
VLE and Learning Centres at all sites; Lecture and Tutorial staff from nursing, therapies and social work faculties; Individual mentorship scheme; Welfare and student support services.

Assessment:
Range of assessments including essays, Powerpoint presentation, poster presentation, MCQ examination, seminar presentations and portfolio.

Fees Info
The fee where stated is an 'INCLUSIVE FEE' for UK/EU students. This fee includes the tuition fee, examination fee and exam board registration fee. You may have to pay extra for materials for the course. If you are under 19 years or you are in receipt of an income related benefit the course and exam fee is FREE. Students aged 60+ years pay 70% of this fee. Students aged 19+ will also have to pay a College Registration Fee of £30 (or £20 if you receive benefit) once per academic year.Materials costs:
Student will be expected to provide a successful CRB check (which may be paid by employer)

Entry Requirement:
This course is for students 19 years old and over. This course is designed for aspiring associate practitioners who are currently employed at least 15 hours a week and at least 18 years of age/ Every application will be assessed on its individual merits, but typically students will have for example, One A-Level, and Five GCSEs grades A*-C with Maths and English, or equivalent; or NVQ Level Three in health or social care related subject; or A relevant kite marked ACCESS to Higher Education course in a relevant subject or AVCE or GNVQ or B/TEC in a health or social care related subject and have support of their employer; candidates whose first language is not English will be required to demonstrate proficiency by attaining a score of 7.0 in the IELTS test (or equivalent); All applicants will be subject to an interview.
